Premium Information:

  1. OPTION A: EMPLOYER FUNDED FOR FAMILY

    Cost: Guaranteed for one year from contract inception.
    Employee and Family – Please contact PVCS for specific quote. Premiums are paid on a monthly basis per employee.

    NOTES:
      All eligible employees must be included.
    Minimum of 25 employees enrolled
    If any opportunity is given to individual eligible employees to opt out of the coverage, then the payroll deduction Fee Schedule shall apply for the entire group.

    Payments: Each month's payment shall consist of the per-employee rate times the maximum number of eligible employees employed the previous month. At the end of each calendar year, a payment review shall be conducted whereby a calculation of the average number of employees is made for the previous year. The total amount paid during the previous year shall equal or exceed the average employee count for the year times 12 (months) times the monthly per employee rate. Any shortage in this figure shall be remitted to PVCS within 30 days of the end of each contract year.

    Materials: Patient pays wholesale cost, as defined in contract, of any materials (eyeglasses or contact lenses) needed.

    Contract Length: Contracts are for one year and renewal discussion will commence approximately 90 days before the end of each contract year. Contracts may be made with an automatic renewal or similar arrangement if the employer so desires.

  2. OPTION B: EMPLOYER FUNDED FOR EMPLOYEE ONLY

    Cost: Guaranteed for one year from contract inception.

    Please contact PVCS for specific quote.

    - Employee -- Premiums are paid on a monthly basis for all eligible employees, paid by employer
    - One dependent -- Premiums are paid on a monthly basis, paid by employee through payroll deduction
    - Family -- Premiums are paid on a monthly basis, paid by employee through payroll deduction

    NOTES:
      Minimum of 25 employees enrolled
    Dependents funded by payroll deduction.
    If any opportunity is given to individual eligible employees to opt out of the coverage, then the payroll deduction Fee Schedule shall apply for the entire group.

    Payment is made monthly and is due at PVCS no later than the last business day of the month prior to the month services are offered.

    Materials: Patient pays wholesale cost, as defined in contract, of any materials (eyeglasses or contact lenses) needed.

    Contract Length: Contracts are for one year. Renewal may be adjusted to fit the specific needs of the employer.

  3. OPTION C: EMPLOYEE FUNDED THROUGH PAYROLL DEDUCTION

    Cost: Guaranteed for one year from contract inception.

    Please contact PVCS for specific quote.

    - Employee -- Premiums are paid on a monthly basis
    - Employee and one dependent -- Premiums are paid on a monthly basis
    - Family -- Premiums are paid on a monthly basis

    NOTES:
      Minimum of 10 employees enrolled
    Participation funded by payroll deduction.

    Materials: Patient pays wholesale cost, as defined in contract, of any materials (eyeglasses or contact lenses) needed.

    Payment is made monthly and is due at PVCS no later than the last business day of the month prior to the month services are offered.

    Length of contract -- Contracts are for one year.

  4. APPLIES TO OPTIONS

    Eligibility for Service. Employees eligibility for service under this plan shall be determined by the employer. The method for communicating to PVCS who is eligible for services will be negotiated with the Plan Administrator with a goal of being convenient to the contracting organization.

    Termination during a plan year. In the event an employee under a payroll deduction plan terminates his/her employment prior to the end of a contract year and has used vision services under this contract, that employee must pay the remainder of his/her annual fee/premium at the time of, or prior to, receiving a final paycheck. If the employee has not used PVCS services and wishes to discontinue the contract, he/she may do so without further fee/premium.

    Utilization Review. Review of overall plan use by each client company will be conducted on a routine basis by PVCS. Reports and PVCS analysis are available, upon request, to each client employer.

    Auditing Privileges. The VisionAdvantage Plan Administrator conducts audits of participating Optometrist’s records on a routine basis to ensure contract compliance. Employers may contact the Plan Administrator to request out-of-cycle audits if they perceive a problem with plan execution. Upon request, contracting organizations may receive copies of those audits applicable to their organizations.
